Job Description:
Contract Type: Flexible(26 weeks to 1 year, or Temp-to-Perm)
Schedule: Full-Time, 40 hours/week
Location: Centralia, Washington and surrounding areas
Local Pay $70/hr
Travel Pay Package:
- Hourly Rate: $45/hour (40 hours/week)
- Meals Stipend (Non-Taxed): $476/week
- Housing Stipend (Non-Taxed): $770/week
- Total Gross Weekly: $3,046
- $500 bonus!
Position Summary:
We are seeking a dedicated and experienced HomeHealth Physical Therapist to join our team in Centralia, WA. This roleis ideal for a compassionate clinician who thrives in a flexible,patient-centered environment. You will provide in-home therapy services topatients recovering from illness, injury, or surgery, helping them regainmobility and independence.
Key Responsibilities:
- Deliver skilled physical therapy services in patients’ homes
- Develop and implement individualized care plans
- Collaborate with interdisciplinary teams to ensure quality care
- Accurately document visits and patient progress using Homecare Homebase (preferred)
- Meet a productivity expectation of 26 points per week
Qualifications:
- Licensed Physical Therapist in the state of Washington (required)
- Minimum 1 year of experience in physical therapy (required)
- Experience with OASIS documentation and Homecare Homebase (preferred)
- Valid Driver’s License and reliable transportation
- Must meet onboarding requirements (see below)

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.
Minimum Requirements:
- Current licensure as a Physical Therapist in the state ofpractice
- Graduate of an accredited school of Physical Therapy
- One (1) year of prior professional Physical Therapyexperience preferred
- Current CPR if applicable
- TB Questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
