The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.
Minimum Requirements:
- Current RN licensure in state practicing
- At least one year of Case Management experience preferred
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
Registered Nurse (RN) Case Manager – Hospice
Location: Charlottesville, VA
Schedule: Monday–Friday, 8:00 AM–4:30 PM | On-call every other weekend
Pay: $50/hour
Mileage Reimbursement: $0.70/mile
Position Overview: We are seeking a compassionate and skilled Registered Nurse to join our hospice team as a Case Manager. This role involves providing care, comfort, and support to terminally ill patients in both home and facility settings across Albemarle County. The RN Case Manager will manage a caseload of approximately 15 patients, conducting 4–5 visits per day, with travel up to one hour from the office.
Key Responsibilities:

Deliver high-quality hospice care in accordance with nursing standards

Make clinical judgments and coordinate patient care with supervision

Maintain accountability for patient outcomes and assist in care management

Provide emotional support and education to patients and families

Document care accurately and timely
About the Role: This is a field-based position requiring travel to patient homes and facilities. Candidates must be comfortable working independently, managing a dynamic caseload, and providing end-of-life care with empathy and professionalism.
