The Long-Term Acute Care Registered Nurse provides planningand delivery of direct and indirect patient care to medically complex patientsin long-term acute settings.
Minimum Requirements:
- Current RN license for the state in which the nursepractices in
- One (1) year nursing experience in a long-term acute carefacility preferred
- Experience in acute rehabilitation for vents, wound care,cardiac, and stable post trauma patients
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
- Preferred backgrounds: LTAC > Stepdown > ICU > ER > Medsurge (Stepdown nurses have the highest success in adaptation)
- Ideal experience: Vents and drips (pressors/sedatives); not mandatory but highly valued.
- Success factors: Enthusiasm and willingness to learn are strong indicators of fit.
- Screening approach: All submissions will be reviewed unless exclusively homecare/medsurg
- Charting system: Epic; patient assignments based on acuity.
- Typical ratios: 4–5 patients per RN; reduced for high-acuity cases (e.g., multiple drips, fresh trachs).
- High Risk Airway policy: Max 3:1 ratio; strict limits on assignments.
- Respiratory Therapist coverage: 7:1 ratio; one per floor, third RT floats.
- Mostly geriatric; common conditions include respiratory failure, ESRD, CHF, HTN.
- ~50% have artificial airways; ~10% on critical drips.
- Dialysis performed bedside; no CRRT.
- Modified self-scheduling with weekend/holiday rotation
- Block scheduling possible if it fits balance and policy.
- Odd scheduling needs (e.g., no Tue/Wed availability) may be accommodated.
- Required modules via Select University – reimbursed on first paycheck
- New 2-day orientation – nurses are assigned to a preceptor, can orient on day shift if preferred
