Job Description This position involves providing nursing care in a home care setting, offering high-quality services tailored to individuals with brain injuries, intellectual and developmental disabilities, substance use disorders, or emotional and behavioral challenges. Key responsibilities include:
Duties and Responsibilities:

Deliver nursing care and personal support in accordance with patient needs within a home care environment.

Conduct health assessments, administer treatments prescribed by physicians, and respond to changes in patient conditions.

Monitor the health conditions of individuals served to ensure high standards of care.

Administer medications, maintain accurate documentation, and oversee staff involved in medication administration.

Implement health care services outlined in Individual Service Plans (ISPs) and evaluate patient outcomes.
-
Train staff on universal precautions, infection control, and medication administration.
-
Serve as a liaison and advocate with medical professionals and external agencies to coordinate consistent care.

Provide educational training for individuals served on topics such as self-medication, prescribed treatments, sexual health, STD prevention, and smoking cessation.

Maintain a structured and supportive home environment to promote patient health and recovery.
Minimum Requirements:
-
Associate’s Degree in Nursing or a related field, or state-approved nursing certification.

At least one year of nursing experience with individuals in the served population.

Active state licensure as an LPN or LVN.
-
A valid driver’s license, car registration, and auto insurance.

State-required certifications, including CPR, First Aid, HIV/AIDS training, BBP, and ACT certification.

Strong communication skills to effectively interact with staff, individuals served, and their families.
