LPN/LVN – Home Health (Night Shift)

Schedule: 36/48 Rotation
Shift: 7:00 PM – 7:00 AM (Night Shift)
Position Type: LPN – Nights
Position Overview
Under the supervision of a Registered Nurse, the Licensed Practical/Vocational Nurse (LPN/LVN) provides skilled, one-on-one nursing care to patients in their homes and may accompany them to appointments when needed. This role requires clinical expertise and the ability to adapt to home health environments.
Minimum Requirements
- Graduate of an accredited practical or vocational nursing program
- Active LPN/LVN license in the state of practice
- Minimum 1 year of professional nursing experience (home care preferred)
- Current CPR certification (if applicable)
- TB Questionnaire, PPD, or chest x-ray (if applicable)
- Current health certificate (per contract or state regulation)
- Must meet all federal, state, and local requirements
- Must be at least 18 years of age
Experience Requirements
- Minimum 2 years of LPN experience
- Comfortable working with ventilators and tracheostomy patients
- Experience in Behavioral Health, Psychiatric care, or Long-Term Care
- Local candidates preferred
