Patient Navigator:
- Travel from facility to facility developing relationships with the patients and the facilities to coordinate patient discharges
- This is essentially a sales job / liaison with the facility
- 3-5 years of experience with discharge planning at a hospital or skilled nursing facility – strongly preferred
- No license required
- Should have a Bachelors in Social Work, Psych, Nursing, etc.
- Interview process
- 2 interviews with the client – First with Lea the coordinator, then with the regional manager
- Pay Structure:
- $85k base salary
- Up to $65k annual bonus based on conversion rates of discharges to patients
- Full time employee eliogible for benefits and start accruing PTO after 90 days.
- Spread:
- $9k over 13 weeks ($692/wk)
