Amergis Healthcare has a fulfilling opportunity for an RN Case Manager to provide addiction services at a health clinic in Lynn, Massachusetts. 

Local Pay: $60/hour, paid weekly

Travel Pay: $2,719/week

Schedule: Monday–Friday, 8:30 AM – 5:00 PM (No weekends or holidays)

Duration: 13 weeks with possible extension 

Qualifications:

  • Active RN license in Massachusetts
  • Experience in community health or nurse case management preferred
  • Bilingual in Spanish, Khmer, or Russian is a plus
  • Strong attendance record and positive work attitude
  • Preferred: Certification in Case Management (CCM) or related specialty
  • Preferred: Experience working with patients in substance use treatment programs
  • Preferred: Familiarity with community resources and support services for individuals with substance use disorders

Job Duties: 

  • Identify and engage patients appropriate for RN Case Management based on diagnosis, treatment regimen, risk status, compliance, ER usage, or grant-specific criteria.
  • Collaborate with PCPs, team RNs, clinical pharmacists, and other providers to develop and revise care plans that support best practices and patient self-management goals.
  • Coordinate referrals and services across departments and external agencies (e.g., Social Services, WIC, Behavioral Health, Dental, Eye Care, Pharmacy).
  • Ensure patients are scheduled for follow-ups, labs, and other necessary visits; provide outreach for missed appointments.
  • Document patient interactions and interventions using EHR/EPM systems, incorporating learning assessments and teaching guidelines.
