RN Case Manager – Addiction Services Program
Location: Lynn, MA
Facility Type: Health Center/Clinic
Schedule: Monday–Friday, 8:30 AM – 5:00 PM (No weekends or holidays)
Duration: 13 weeks to start, with strong potential for extension
Compensation:
- Local: $60/hour
- Travel: $2,719/week
Position Overview
The RN Case Manager plays a vital role in coordinating comprehensive care for patients with substance use disorders. This position focuses on developing individualized care plans that address each patient’s medical, psychological, and social needs. Working closely with a multidisciplinary team, the RN Case Manager ensures patients have access to essential resources and services that support recovery and improve overall health outcomes.
By providing education and ongoing support, the RN Case Manager empowers patients to actively participate in their treatment journey. The ultimate goal is to enhance quality of life, reduce barriers to care, and foster a supportive environment for long-term recovery.
Key Responsibilities
- Identify and engage patients appropriate for RN Case Management based on diagnosis, treatment regimen, risk status, compliance, ER usage, or grant-specific criteria.
- Collaborate with PCPs, team RNs, clinical pharmacists, and other providers to develop and revise care plans that support best practices and patient self-management goals.
- Coordinate referrals and services across departments and external agencies (e.g., Social Services, WIC, Behavioral Health, Dental, Eye Care, Pharmacy).
- Ensure patients are scheduled for follow-ups, labs, and other necessary visits; provide outreach for missed appointments.
- Document patient interactions and interventions using EHR/EPM systems, incorporating learning assessments and teaching guidelines.
Qualifications
Minimum Requirements:
- Active RN license in Massachusetts
- Experience in community health or nurse case management preferred
- Bilingual in Spanish, Khmer, or Russian is a plus
- Strong attendance record and positive work attitude
Preferred Qualifications:
- Certification in Case Management (CCM) or related specialty
- Experience working with patients in substance use treatment programs
- Familiarity with community resources and support services for individuals with substance use disorders
