Seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) or Licensed Marriage & Family Therapist (LMFT) to join the Integrative Behavioral Health Services team. This role involves conducting assessments, developing treatment plans, and providing supportive counseling, crisis intervention, and brief therapy to diverse patient populations.
Key Responsibilities:
- Conduct mental health screenings and evidence-based psychotherapy.
- Collaborate with primary care providers to improve patient outcomes.
- Facilitate referrals to community resources and specialty care.
- Provide individual, family, couple, and group therapy.
- Participate in warm handoffs and urgent evaluations.
- Maintain accurate documentation and participate in case management.
Qualifications:
- Licensed as LCSW or LMFT in California.
- Master’s degree in Counseling or related field.
- Experience in community health preferred.
- Bilingual in English and Spanish strongly preferred.
- Familiarity with multicultural populations, especially Latino communities.
- Proficient in EPIC systems and documentation or willing to learn
Work Schedule:
- Monday–Friday, with occasional evening or Saturday shifts.
- Potential for Hybrid after training period completed
- Part Time of at least 3 days can be accepted
Success Metrics:
- Timely documentation and billing.
- Meeting patient visit benchmarks.
- Active participation in warm handoffs and team collaboration.
