RN Case Manager

The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

Temple Rehabilitation Hospital (Lifepoint Health)
The RN Case Manager:
  • Coordinates care management to ensure optimal use of resources and compliance with regulations.
  • Conducts comprehensive assessments, develops care plans, and evaluates patient progress.
  • Integrates case management, utilization review, and discharge planning to enhance care quality and cost-effectiveness.
  • Acts as a patient advocate, addressing holistic needs and facilitating resource utilization and discharge planning.
  • Communicates with patients and families in an interdisciplinary environment, ensuring care aligns with professional and ethical standards

Everest Rehabilitation Hospital
While specific details were limited, RN Case Managers there are similarly responsible for:
  • Assessing and addressing psycho-social needs, especially for patients with complex medical conditions.
  • Providing discharge planning, counseling, and coordinating services across the continuum of care.
Qualifications Typically Required
  • Active RN license (or other healthcare licensure like Social Work, PT, OT, etc.).
  • Preferred certifications: CCM, ACM, RN-BC.
  • Experience in inpatient or rehabilitation settings.
  • Strong communication, organizational, and analytical skills.
