  • Responsibilities:
  • Will be operating in a Case Manager capacity versus hands on work (plan of care, ordering medication, etc.)
  • Will be tasked in visiting hospice patients at various locations (patients homes, LTC/Assisted Living Facilities, etc.).
  • The one day on the weekend is basically on-call, taking triage calls
  • Ordering meds/supplies
  • Will have to drive their own vehicle (milage reimbursed)
  • Will not need to drive patient

Notes: Two years of recent experience in community-based hospice work.
