Job Description:

Client Assessment and Diagnosis

• Conduct comprehensive assessments to diagnose mental health conditions.

• Utilize standardized assessment tools and clinical interviews.

• Develop individualized treatment plans based on client needs and diagnosis.

Therapeutic Interventions

• Provide individual, group, and family therapy sessions.

• Implement evidence-based therapeutic approaches (CBT, DBT, EMDR, etc.).

• Monitor and documents client profess and adjust treatment plans as needed.

Case Management

• Coordinate with other healthcare providers, including psychiatrists, social workers, physicians, and

rehabilitation specialists/counselors in the treatment of a client.

• Refer clients to appropriate community resources and support services.

• Advocate for clients within the healthcare system and community.

Documentation and Reporting

• Maintain accurate and timely clinical records in compliance with legal and ethical standards.

• Prepare and submit required reports and documentation to regulatory agencies.

• Ensure confidentiality and privacy of client information.

The Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention.  The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span. 

Minimum Requirements:

• Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education required
• Active license as an LCSW in state of practice required
• Minimum of one year of social work experience preferred
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable 
• Current Health certificate (per contract or state regulation) 
• Must meet all federal, state and local requirements 
• Must be at least 18 years of age
