MUST HAVE OR BE WILLING TO OBTAIN A UTAH LICENSE (OT DOPL)
Contract Details: This contract is located in St. George, UT and will last through the entirety of the 25/26 school year. This position is open to candidates with a pending license. The role will involve supporting five elementary school sites located in close proximity to one another. The provider will typically be scheduled at one to two locations per day. Daily responsibilities include setting IEP goals, conducting case assessments, and delivering direct services to students.
Local Contract Estimate: $58 – 62 / hour
Travel Contract Estimate: $2,500 – $2,700 / week
(Weekly Travel Rate Available for Candidates 50+ miles away)

Minimum Requirements:
- Current licensure as an Occupational Therapist in the State of practice
- One (1) year of prior professional Occupational Therapy experience preferred
- Current CPR if applicable
- TB Questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state and local requirements
