Job Title: PRN Rehab LPN – Neuro/Spinal Cord Focus (Part-Time)
Location: O’Fallon, MO
Start Date: ASAP
Contract Length: 13 Weeks
Schedule: 1 shift/week | 7:00 AM – 7:30 PM (12 hrs/week)
Pay Rate: $38/hour
Job Description:
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join a specialized rehab team in O’Fallon, MO. This role focuses on neuro and spinal cord injury recovery, helping patients regain independence in daily activities such as walking, cooking, and cleaning.
Key Responsibilities:
- Provide direct care to neuro rehab patients, including stroke recovery
- Oversee and support a team of CNAs
- Utilize Epic for charting (strongly preferred)
- Occasionally transport patients to appointments or errands (vehicle provided, insured, and accompanied by CNA or OT)
Requirements:
- Valid LPN license
- 2+ years of Rehab or Acute Care experience (neuro focus preferred)
- Experience with spinal cord injuries and stroke recovery
- Epic charting experience highly recommended
- Valid driver’s license (required)
- Must be at least 18 years old

The Long Term Care LPN assumes responsibility andaccountability for the delivery of care for patients in a Long TermCare/Assisted Living Facility. The nurse demonstrates ability to make clinicaljudgments in an effective and efficient manner with supervision, as well ascritical thinking and performance ability in the coordination of patient care.Additionally, this individual performs according to nursing standards and isaccountable in managing patient care and assisting others in the management ofpatient care.
Minimum Requirements:
- Current Licensed Practical Nurse License for the state inwhich the nurse practices
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray
- One year prior professional nursing experience preferred
- Current BLS card
- Successful completion of screening tests for the clinicalspecialty area(s) in which the LPN practices with a score of 80% or greater
