As a Homecare LPN, you willwork collaboratively with Registered Nurses and interdisciplinary teams toprovide skilled nursing services, promote patient independence, and supportoverall health and well-being. This position is ideal for nurses who arepassionate about personalized care and making a meaningful impact in the livesof older adults and individuals with complex health needs.

Key Responsibilities:

• Deliver direct nursing care, including medication administration, wound care, and monitoring of vital signs, in accordance with physician orders and care plans
• Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support
• Observe and report changes in patient condition to the supervising RN or healthcare provider
• Educate patients and caregivers on disease management, medication adherence, and health maintenance
• Provide emotional support and build trusting relationships with patients and their families
• Collaborate with the care team to ensure continuity and quality of care
• Maintain accurate and timely documentation in compliance with state and agency standards
• Adhere to infection control protocols and safety procedures in the home environment

Qualifications:

• Current and active LPN license in the state of Virginia
• Current and active BLS
• TB screening (PPD, chest x-ray, or questionnaire as applicable)
• Current health certificate (as required by contract or state regulation)
• Must meet all federal, state, and local employment requirements
• Must be at least 18 years of age
• Must be willing to travel and relocate for the duration of the contract
