Amergis Healthcare has an opportunity for a Clinical Assessment RN to provide services between Boston (north of I-90 and Malden). Assessments will be conducted in residential (in-home) settings with mostly geriatric patients)
Schedule: Monday–Friday, 8:30 AM – 5:00 PM (no weekends, holidays or on-call)
Caseload: 3 assessments per day (12/week)
Compensation:
  • Local: $57/hour
  • Travel: $2,600/week
  • Mileage Reimbursement: $0.70/mile
  • Parking Reimbursed
Position Overview:
The Assessment Nurse conducts in-person clinical assessments for members to evaluate their health and functional needs. This role supports care planning by identifying unmet needs and collaborating with interdisciplinary teams to improve member outcomes.

Key Responsibilities:
  • Perform in-home assessments and clinical screenings using tools like the Minimum Data Set (MDS), PHQ-9, and others.
  • Conduct medication reconciliation, assess adherence, and provide disease education.
  • Escalate urgent concerns and refer members to appropriate services (e.g., behavioral health, LTSS).
  • Document findings in electronic health records and care management platforms.
  • Support telehealth assessments when appropriate.
  • Collaborate with care teams to develop individualized care plans and determine program placement.
  • EMR is ClinicalWorks.
Required Qualifications:
  • Must have experience with MDS (or Oasis) 
  • Active RN license in Massachusetts
  • 1+ years of clinical experience (including clinical rotations)
  • Strong clinical judgment, organizational, and communication skills
  • Proficiency in Microsoft Office and willingness to use telehealth tools

Preferred Qualifications:
  • Bachelor’s Degree in Nursing
  • Experience with electronic medical records
  • Background in care management
  • Bilingual in English and a second language
