Licensed Clinical Social Worker – Pediatric Hospital – Canton, MA

 Amergis Healthcare is looking for a Licensed Clinical Social Worker that will collaborate with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention.  The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span.

Position Details:

• Location: Canton, MA
• Setting: Pediatric Hospital
• Schedule: Mon-Fri 8a-4p or 9a-5p
• Compensation: $60/hr LCSW, $63/hr LICSW

ASSESSMENT OF PATIENTS          

• Assesses competency levels of pediatric patients (18 to 22 years old) to determine need for guardianship.
•  Interviews patients and families at a time of pre-admission evaluation to assess appropriateness for admission.  Documents findings and makes recommendation to the Admission Committee. 
• Meets with family of each new patient, obtain a complete psycho-social history which is placed in patient’s medical record.
• Coordinates and monitors assigned program activities in order to ensure effective operations and compliance to established standards.
• Reviews and analyzes data on a continuous basis through the care planning process to determine progress by measuring outcomes to make recommendations for changes in procedures affecting guidelines, interventions and approaches that accomplish program/treatment objectives.
• Evaluates information about patient in order to suggest approaches for resolving legal, family, social, economic, behavioral and/or physical problems to assist or motivates patients in solving problems , to determine patient’s progress and to inform patients about their eligibility, status etc.
• Communicates with various case management providers in order to make evaluations concerning patient’s suitability as referral or placement sources in connections with agency program. 
• Based on assignment, will serve as a member of an assessment team or treatment team.
• Provides technical assistance to employees regarding the assessment of patient’s needs and the determination of proper treatment plans.
• Responds to inquiries from agency staff and others in order to provide information concerning assigned agency programs.
• Maintains liaison with various private, local, state and federal agencies and others in order to exchange information and/or resolve problems.
• Performs related job duties such as attending meetings and conferences, maintaining records; and preparing reports.
• Takes part as a member of the treatment team in the completion of the PRHC integrated assessment.
• Provides on the job training and orientations to employees.
• Develop and implement procedures and guidelines to accomplish assigned agency program objectives and goals.
• Review reports, memoranda etc. for completeness, accuracy and content.
• Confer with management staff and other to provide information concerning program implementation, evaluation and monitoring to define the purpose and scope of proposed programs.
• Oversees and monitor activities of the assigned unit.
• Participate in monthly family support.

CARE OF PATIENT

• Conducts group therapy with patients to assist them in understanding and developing coping mechanisms regarding their physical disabilities and relationship issues.
• Monitors and evaluates the progress of patients receiving services through on site visits, discussion with service providers, review of progress of progress reports, etc. in order to determine if placements are appropriately meeting the patient’s needs.
• Maintains and liaison with various public and private agencies, organizations and officials in other to exchange information and/or to develop or improve services and placement opportunities for patients.
• Confers with agency staff members in order to obtain and exchange information concerning clients, agency services and to resolve problems.
• Makes determinations of suspected, alleged or reported violations of agency standards, regulations etc. and recommends appropriate actions
• Based on assignment, may testify at hearings in order to provide information concerning clients, agency services and to resolve problems.
• Provides direct counseling to patients and their families as needed in order to assist patient with adjustment to PRHC and to help family cope with difficulties care for a disabled child.
• Develops, implements and modifies, as needed, appropriate long and short-term treatment/intervention programs as related to school-aged, adolescent and your adult populations the physical disabilities.
• Able to demonstrate knowledge and skills regarding age specific physiological changes, developmental needs, equipment needs and appropriate therapeutic interventions.
• Knowledge of principles of growth and development over the life span and interpret the appropriate information needed to identify each patients requirements related to age specific needs.
• Coordinates and implements social skills psycho-educational programming groups for young adult hospital patients preparing to transition to community.
• Provides conflict resolution strategies as part of peer mediation programming for disabled adolescents and young adults in a hospital setting.

PATIENT/FAMILY EDUCATION

• Provides patient and family therapy to assist patients and their family members to acquire a deeper understanding  of patient’s physical disability and its impact upon the family.
• Assist patients and families in discharge planning for patient in order to ensure a smooth transition in the community upon discharge.
• Explore housing, educational and medical options in the community with the patient and family in order to provide best possible plan for patient upon discharge.
• Participate in patient/family education in the areas of treatment planning, process and progress adult service providers and insurance information.

OTHER

• Participates in Quality Improvement Program.
• Demonstrates critical thinking and sound judgement in clinical and administrative decision making.
• Reviews and analyzes available data to keep abreast of developments in program planning concepts, treatment of  other methodologies, legal requirements and related areas in order to recommend alternate course of treatment of appropriate actions.
• Prioritizes work in order to meet needs/expectations (outcomes).

Qualifications             

• Need to have experience delivering psychological services to children, adolescents, and young adults with intellectual disabilities as this is the bulk of their population at the moment.
• Must be licensed (LICSW preferred)
