Location: Stillwater Center, Dayton, OH
Facility Type: Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID)
Shift: Monday–Friday, Day Shift (typically 8:00 AM – 4:30 PM)
Duration: Contract (with potential for extension)
Amergis Healthcare is seeking a compassionate and experienced Physical Therapist (PT) to join our team at Stillwater Center, a county-operated facility serving individuals with profound developmental disabilities and complex medical needs. The PT will play a vital role in improving mobility, function, and quality of life for residents through individualized therapy plans.
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
- Deliver therapeutic interventions to improve strength, mobility, balance, and coordination
- Collaborate with interdisciplinary teams including nursing, occupational therapy, and speech therapy
- Document patient progress and outcomes in accordance with facility and regulatory standards
- Educate staff and caregivers on mobility techniques and assistive devices
- Participate in care planning meetings and contribute to long-term care strategies
- Active Ohio Physical Therapist License
- Minimum 1 year of experience in a long-term care, SNF, or ICF/IID setting preferred
- Strong understanding of developmental disabilities and neurological conditions
- CPR certification (BLS)
- Excellent communication and documentation skills
- Serve a vulnerable and inspiring population
- Work in a collaborative, multidisciplinary environment
- Enjoy consistent daytime hours with no weekends or holidays required
