Job Title: CertifiedNursing Assistant (CNA) – Homecare

Location: InnovAge- Charlottesville, VA

Job Type: Full-Time/ Contract (Assignment length may vary based on client needs)

Job Description:

Join a compassionate and skilled home care team dedicatedto supporting patients in the comfort of their own homes. As a Homecare CNA,you’ll work under the supervision of a Registered Nurse and be responsible ofproviding essential personal care services tailored to each patient’s needs.This role is ideal for individuals who are passionate about making a meaningfuldifference in the lives of others through hands-on care and support.

Key Responsibilities:

  • Assist patients with daily living activities such as bathing, dressing, grooming, and mobility
  • Monitor and report changes in patient condition to supervising clinicians
  • Support patients with light housekeeping, meal preparation, and medication reminders
  • Provide companionship and emotional support to patients and their families
  • Follow care plans and safety protocols as directed by supervising healthcare professionals
  • Maintain accurate documentation of care provided

Qualifications:

  • Certification as a CNA in the state of Virginia
  • Current and active BLS
  • TB screening (PPD, chest x-ray, or questionnaire as applicable)
  • Current health certificate (as required by contract or state regulation)
  • Must meet all federal, state, and local employment requirements
  • Must be at least 18 years of age
  • Must have reliable transportation
