RN Case Manager – Residential Setting

Job Summary:

The RN Case Manager in a residential setting is responsiblefor coordinating and overseeing the delivery of healthcare services toresidents. This role ensures that each resident receives individualized,high-quality, and cost-effective care. The RN Case Manager acts as a liaisonbetween residents, families, healthcare providers, and support staff to promoteoptimal health outcomes and continuity of care.

Key Responsibilities:

• Assessment & Care Planning:
• Conduct comprehensive health assessments upon admission and at regular intervals.
• Develop, implement, and regularly update individualized care plans in collaboration with interdisciplinary teams.
• Care Coordination:
• Coordinate medical appointments, treatments, and follow-ups.
• Communicate with physicians, therapists, and other healthcare providers to ensure cohesive care delivery.
• Facilitate transitions of care, including hospital admissions and discharges.
• Advocacy & Support:
• Advocate for residents’ healthcare needs and preferences.
• Educate residents and families on health conditions, medications, and self-care strategies.
• Provide emotional support and counseling as needed.
• Compliance & Documentation:
• Maintain accurate and timely documentation in accordance with regulatory standards.
• Ensure compliance with state and federal healthcare regulations and facility policies.
• Team Collaboration:
• Participate in interdisciplinary team meetings and case conferences.
• Train and support residential staff on health-related procedures and protocols.
• Discharge Planning:
• Initiate discharge planning upon admission.
• Coordinate with community resources and services to ensure a smooth transition post-discharge.

Qualifications:

• Current RN license in the state of practice.
• Minimum of 2 years of nursing experience, preferably in case management or residential care.
• Strong knowledge of chronic disease management, mental health, and community resources.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work independently and collaboratively in a team environment.
