Community Health Worker - $23/hr

Community Health Worker (CHW) meet with clients to assist with applying for social service assistance, provide referrals for housing, food, social, transportation, and other community resources.  They are bound by HIPAA because one of their primary purposes in assisting the clients is to ensure they have access to health care.  They help identify possible barriers and increase opportunities for access to multiple state agency services.

Minimum Requirements:

• High School diploma or its equivalent
• Current CPR if applicable
• TB Questionnaire, PPD or chest x-ray if applicable   
• Must meet all federal, state and local requirements 
• Must be at least 18 years of age

M-F Days

On-going Contract

$23/hour Pay Rate.

$0.70 per mile milage reimbursement.

Reliable vehicle due to some traveling involved on day to day basis.

High School Diploma

Applicable experience as a Community Health worker type role.
