Job Title: Physical Therapist

Location: Fort Walton Beach, FL

Job Description: We are seeking a dedicated and skilled Physical Therapist to join our team in Fort Walton Beach, FL. The Physical Therapist will be responsible for performing patient evaluations, providing therapy services, and documenting these services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.

Key Responsibilities:

• Conduct patient evaluations to determine appropriate treatment plans.
• Provide therapy services as outlined in the plan of care.
• Document therapy services and patient progress accurately.
• Collaborate with physicians and other healthcare professionals to ensure optimal patient care.

Minimum Requirements:

• Current licensure as a Physical Therapist in the state of Florida.
• Graduate of an accredited school of Physical Therapy.
• One (1) year of prior professional Physical Therapy experience preferred.
• Current CPR certification, if applicable.
• TB Questionnaire, PPD or chest x-ray, if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state, and local requirements.
• Must be at least 18 years of age.

General Comments:

• Schedule: Monday – Friday, guaranteed 37 hours per week for 13 weeks+ with nights until 7-8 PM (location specific).
• License: Must hold a valid Physical Therapy license in Florida.
• Experience: Minimum of one recent outpatient travel assignment. One or more years of recent outpatient orthopedic experience preferred.
• Patient Volume: 10-13 patients per day, with 1-hour evaluations and 1:1 visits lasting 30-45 minutes.
