Amergis Is seeking highly skilled RN LTAC for a position in Centralia, WA 98531.

Position Summary:

This role is responsible for assessing the health needs and goals of residents and their families or guardians, implementing a nursing plan of care in collaboration with interdisciplinary team members and healthcare providers, and delivering professional nursing care in accordance with all applicable state licensure and regulatory requirements. The position also provides clinical oversight of CNAs.

Essential Functions:

  • Perform systematic assessments focusing on the physiological, cultural, psychological, spiritual, and cognitive status of residents.
  • Evaluate, develop, and prioritize a plan of care based on residents’ individualized care plans and nursing standards of care.
  • Provide safe, therapeutic care in a knowledgeable, skillful, and continuous manner involving all members of the healthcare team.
  • Demonstrate competence in the care of pediatric and young adults with developmental disabilities and medically complex needs.
  • Assess and evaluate residents’ plans of care and teaching needs, document effectiveness, and revise plans as needed.
  • Administer and document medications, treatments, diets, and other interventions safely and as outlined in residents’ care plans and medical records.
  • Observe residents for changes in condition, consult and collaborate with residents’ providers and healthcare professionals as needed, and document changes and interventions in residents’ records.
  • Establish rapport with family members and guardians, assist and instruct them in nursing care procedures as appropriate.
  • Accompany residents to non-virtual medical and health-related appointments, provide appropriate medical information to healthcare professionals, and collaborate on care.
  • Order resident supplies, equipment, and medications as needed.
  • Oversee care provided by CNAs within their scope of practice.
  • Adhere to HIPAA and all state and federal medical privacy laws.
  • Perform other duties as assigned.

  • Adherence to organizational policies and procedures, including infection prevention and control and general safety precautions.
  • Ability to demonstrate sensitivity and respect to community members from multiple cultures and to individuals who experience disabilities.

Work Environment: This role operates in a home healthcare facility and may include accompanying residents to outside medical and health-related appointments. The position requires flexibility and willingness to work nights, weekends, and holidays. On-call duties are shared on a rotating schedule. Per Diem status requires availability for 4 shifts per month. The environment may include exposure to communicable diseases, bodily fluids, and other potential hazards. Nursing staff must wear Personal Protective Equipment (PPE) as required and follow strict guidelines to protect themselves and residents from infectious diseases; use proper lifting techniques at all times and seek help as needed.

Physical and Sensory Demands:

  • Position requires a full range of motion. Must be able to walk, bend, stretch, lift, and stand.
  • Must be able to grasp and use fine hand coordination.
  • Must be able to distinguish between smells, hear audible sounds, lift 40 lbs, and operate a car safely.

Qualifications:

  • Degree from an accredited School of Nursing.
  • Current Registered Nursing License with the WA State Department of Health.
  • Must pass a State and Federal Background Check, including fingerprints.
  • Valid State Driver’s License.
  • Must maintain current vehicle insurance.
  • Current Healthcare Provider BLS/CPR certification.
  • Current Food Handler’s Permit or obtain within 30 days of hire.
  • Proof of licensing-required vaccinations.
  • Abuse Prevention Systems Sexual Abuse Awareness Training within one year of hire.
  • DDA Developmental Disabilities Specialty Training within one year of hire.
  • Must acquire and provide proof of 30 hours of CEUs per year.

Position Description:

The Long-Term Acute Care Registered Nurse provides planning and delivery of direct and indirect patient care to medically complex patients in long-term acute settings.

Minimum Requirements:

  • Current RN license for the state of practice.
  • One year of nursing experience in a long-term acute care facility preferred.
  • Experience in acute rehabilitation for vents, wound care, cardiac, and stable post-trauma patients.
  • Current CPR certification (if applicable).
  • TB questionnaire, PPD, or chest x-ray (if applicable).
  • Current Health Certificate (per contract or state regulation).
  • Must meet all federal, state, and local requirements.
  • Must be at least 18 years of age.

General Comments:

  • 1 Noc (most urgent) and 2 Day 3×12 positions must be open to any days of the week, including the potential for all weekends.
  • Vent and trach experience required; please highlight on the cover sheet.
  • Looking for a minimum of 1 year of experience with pediatrics.
  • DNS is asking to interview candidates prior to them coming onsite.
  • Submission packet should include a cover sheet, resume, and license (include details on pediatric and trach/vent experience).
  • Flu shot required (medical exemption accepted with a doctor’s note stating the nurse has severe adverse reactions).
  • Pediatric and young adult, DDA experience, vent and trach experience required. Most residents are medically complex and often go to Mary Bridge or Seattle Children’s as the local hospital is not equipped to care for them.

Pay Package:

·         Travel Pay Rate: $37.65/hr x 36hrs= $1,355.40wk

·         Weekly Allowances: $1,246 (Meals $476wk and Housing $770wk)

·         Total Weekly Paycheck Gross: $2,601.40wk
