Shift/Schedule: Monday-Friday, 8/8:30a-2:30-3p,30-35 hours/week

Summary:

  •  – Responsible for implementing the assigned participant’s Person Centered Plan (PCP) working with the participant and their family to improve socialization, self-help, communication and pre-vocational skills. The Direct Support Professional – 1:1 will work with an assigned participant at the organization and in the community

 

Essential Duties andResponsibilities:

  • Communicate with participant, families, professional staff and community members in a person-centered manner
  • Develop a positive working relationship with assigned participant and family/caregiver
  • Provide community-based developmental vocational habilitation services consistent with participant’s PCP
  • Develop good working knowledge of participant’s Behavior Plan, utilizing strategies and techniques within the plan to assist participant with decreasing target behaviors/increasing communication skills
  • Implement skill acquisition in areas of , daily living skills, socialization, leisure, basic academics, motor skills, communication, and self-advocacy
  • Participate in the development, implementation, monitoring and updating of assigned participant’s PCP
  • Assist assigned participant with toileting and personal hygiene as needed
  • Maintain complete and accurate participant daily data entry in ICare, tracking behavior data, and in other required documentation

 

Other Duties andResponsibilities:

  • Provide transportation for assigned participant, which will include personal vehicle or company vehicles.
  • Provide mobility monitoring and/or training, as needed.
  • Assist in the completion of all quality assurance protocols including service satisfaction surveys, program efficiency and outcome measurements.
  • Additional responsibilities as assigned by the Manager, Day Habilitation
  • Demonstrate the 5 Core Values (Accountability, Participant Focus, Quality, Independence and Mutual Respect)

 

Qualification Requirements:

  • Minimum age of 18.
  • Access to reliable automobile with valid insurance, and a valid driver’s license with no more than 3 points.
  • Must possess basic computer skills including Microsoft Word and Outlook
  • Must possess excellent written and verbal communication skills
