Amergis Healthcare Staffing is seeking a Wound Care RN.  The Wound Care RN, assumes responsibility and accountability for the application of the nursing process and the delivery of patient care within Skilled Nursing Facilities. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision. The nurse demonstrates critical thinking and performance ability in the coordination of patient care. Consistently performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.

 

Position: RN Wound Care Case Manager (Facility-Based)

Location: Baltimore, MD

  • Territory: 21229, 21230, 21223 (Irvington, Southwest Baltimore, Federal Hill, Locust Point, Carrollton Ridge, South Baltimore)

 

Shift: Full Time, Days, 8a-4:30/5p, 40 hours/week, Monday-Friday

 

Position Type: Direct Placement

Start Date: ASAP

Pay Structure: $95k-100k salary base + $25k-37.5k/quarter ($100k-150k/yr) bonus based on performance on capturing discharges and patient advocacy

 

Position Details:

  • RNs will be based in a Skilled Nursing setting
  • Will be working alongside a Nurse Practitioner. The NPs are seeing 4-5 patients per day, all of which could be in the same SNF within the area. At most, the RN will be traveling to 5 different locations per day
  • About 50% of the day to day responsibilities will be admin related work:
    • Working with the facilities social workers and discharge planners
    • Coordinating and scheduling with the providers and core team
    • Working with the providers on administrative work to assist with patient care (checklists, care plans, benefit navigation, discharges to home care, etc.)
  • Assess and evaluate patient wounds to determine appropriate treatment plans.
  • Develop and implement individualized wound care plans.
  • Clean and dress wounds, ensuring sterile techniques are followed.
  • Monitor and document patient progress, updating treatment plans as necessary.
  • Educate patients and their families on wound care best practices and infection prevention.
  • Collaborate with a multidisciplinary team, including physicians and other healthcare professionals, to optimize patient care.
  • Perform debridement and other wound care procedures as required.
  • Stay updated with the latest wound care research, techniques, and products.
  • Manage wound care supplies and ensure their availability.
  • Provide emotional support and education to patients and their families.
  • Conduct regular follow-up appointments to monitor healing and make necessary adjustments to care plans.

 

Experience/Qualifications:

  • RN License (MD or Compact)
  • Ability to and willingness to travel and treat patients within various Skilled Nursing Facilities within Baltimore, MD
  • Reliable transportation
  • New grad RNs considered!

    • LTC
    • Home Health
    • Case Management
    • Care Coordination
  • Comfortable with productivity metrics RN staff will have productivity metrics to meet on a weekly basis. Standard week is 20 visits. Typically 4-5 visits per day. Ability to meet productivity standards is the most important part of this role.
