RN Case Manager/Discharge Planner | Haverhill, MA

Amergis Healthcare Staffing is partnered with a much a respected facility in Haverhill, MA who is currently seeking a qualified RN Case Manager/Discharge Planner to assist them on a contractual basis.

Details of the position include:

Location: Haverhill, MA

Position: RN Case Manager/Discharge Planner

Duration: 

·       8 weeks to start

·       Option for ongoing duration

·       Temp to perm option available – discussed directly with facility

Schedule:

·       40 hours per week

·       Monday – Friday

·       8:30a – 5:00p

Contract Requirements:

·       Minimum 3 years of experience as an RN

·       Recent or current case management experience in a rehabilitation setting

·       Familiar with discharge planning and Medicare guidelines

·       Familiar with case management standards/guidelines

·       EMR experience and familiarity
