The Amergis Division iscurrently seeking highly skilled Certified/RN at Assured HomeHealth for a position in Spokane, WA 99201.

Position Announcement: (RN) – Home Health

Under the supervision of a RegisteredNurse – Home Health provides skilled one-on-one nursing care to patients intheir homes and accompanies them to appointments when indicated.

Minimum Requirements:

·        Graduateof an accredited practical or vocational nursing program

·        Currentlylicensed as a Licensed Practical/Vocational Nurse in the state of practice

·        Oneyear of prior professional nursing experience, home care experience preferred

·        CurrentCPR certification (if applicable)

·        TBquestionnaire, PPD, or chest x-ray (if applicable)

·        CurrentHealth Certificate (per contract or state regulation)

·        Mustmeet all federal, state, and local requirements

·        Mustbe at least 18 years of age

Position Details:

·        Modality: RN

·        Specialty: Home Health

·        Exclusive to Amergis: Yes

·        Location: Greater Spokane, Washington area

·        Number of Openings: 1

·        Start Date: ASAP, most likely April 28th

·        Length of Contract: 26 weeks

·        Shift: Flexible schedule, daytime Mondayto Friday preferred

·        Hours/Week: 40 hours

Submission Requirements:

·        Atleast 1 year of home health experience required

·        Resume

·        License

Pay Package:

·        Travel Pay Rate: $41/hr x 40hrs= $1,640wk

·        Weekly Allowances: $1,407 (Meals $525wk and Housing$882wk)

·        Total Weekly Paycheck Gross: $3,047wk 

Notes:

·        Candidatewill be required to visit 5-6 patients per day, necessitating a reliablevehicle.

·        Mileagereimbursement included at $0.655/mile.

·        Upto 1 month of training included.

·        Potentialon-call duty once or twice a month at $4.50/hr.

Responsibilities:

·               Provides clinical services within the scope ofpractice, as defined by the state laws governing the practice of nursing, inaccordance with the plan of care, and in coordination with other members of thehealth care team

·       Makes the initial and| or comprehensive nursingevaluation visit, determines primary focus of care, develops the plan of carewithin State specific guidelines, and submits accurate, complete, and timelydocumentation, per policy

·       Regularly evaluates and re-evaluates (as warranted bychanges in condition but at least every 60 days) the patient’s nursing needs

·       Performs patient comprehensive assessments atdesignated time points and develops the appropriate POC, in collaboration withphysician orders

·       Ensures patients meet home health eligibility andmedical necessity guidelines as defined by payer source

·       Initiates, develops, implements and makes necessaryrevisions to the plan of care in collaboration with the physician and otherhealth care professional’s involved in care
