Join Our Team as a Clinical Outreach Specialist!

Are you passionate about making a difference in the lives of homeless individuals, couples, and families? We are offering an exciting opportunity to provide direct services through street outreach and case management, with the opportunity to earn over $70K/year. 

Pay: $35/hour + Mileage Reimbursement

Schedule: Monday – Friday 8am – 5pm (must be flexible on hours in case there is a crisis)

Location: Escondido/Carlsbad/Oceanside

Job Description: Clinical Outreach Specialist

Role: Provide direct services to homeless individuals, couples, and families through street outreach and case management to support housing and stability.

Responsibilities:

  • Work 40 hours/week, with flexibility for weekends/evenings; 80% fieldwork.
  • Re-engage and manage case load of 10-25 participants, focusing on housing goals.
  • Use personal vehicle for community visits (mileage reimbursed).
  • Collaborate with law enforcement and city staff.
  • Follow the City’s Homelessness Action Plan.
  • Connect clients to community-based treatment programs.
  • Conduct assessments and create service plans for long-term housing.
  • Maintain accurate documentation and participate in required trainings and meetings.

Qualifications: 

  • 4+ years of case management experience (required)
  • Experience with severely mentally ill, medically fragile, and homeless populations (required)
  • Bilingual in English and Spanish (preferred)
  • Bachelor’s degree in Social Work or related field (required)
  • Master’s degree in Social Work or related field (required)
  • Registered Associate with CA BBS, working towards licensure (preferred)
