The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.
Minimum Requirements:
Current licensure as a Physical Therapist in the state of practice
Graduate of an accredited school of Physical Therapy
One (1) year of prior professional Physical Therapy experience preferred
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age

The Rochester Psychiatric Center is in need of a full time NYS licensed Physical Therapist (PT). The person will perform various physical therapy services onsite at the facility to include: 1) Plan and conduct medically prescribed treatment of patients with physical disabilities in a group or 1:1 setting; 2) Acts as a team member who coordinates physical therapy services with other disciplines; 3) Attend morning report, Treatment Plan meetings; Barriers to Discharge meetings, and Fall Prevention Committee. The PT will be responsible for upholding documentation standards and meeting these requirements in a timely manner including: PT screening, evaluation, treatment team addendums, progress notes, discharge summaries, ISR comments, etc.
Contractor should have at least experience one year (1) within the past five (5) years and possession of a license and current registration to practice PT in NYS. Prefer that the PT have experience with PT disabilities, fall prevention, and geriatric care.
