The Long-Term Acute Care Registered Nurse provides planningand delivery of direct and indirect patient care to medically complex patientsin long-term acute settings.
Essential Duties and Responsibilities:
- Provides planning and delivery of direct and indirectpatient care
- Responsible for providing care to medically complex patients
- Supervises care, treatment, and services of the patient(s)
- Documents the patient’s plan of care using identifiednursing diagnosis, expected patient outcomes and selected nursing interventions
- Maintains confidentiality at all times with patients,medical staff and co-workers
- Provides a safe, comfortable and therapeutic patientenvironment
- Adheres to and supports thepolicies/procedures/goals/objectives of the hospital in an attempt to providequality patient care
- Revises plan of care according to evaluation, changes inplan of care and nursing interventions
- Performs interventions according to identified priorities,plan of care, and facility policies and patient care outcome standards
- Identifies and prioritizes actions based upon patient carerequirements
Minimum Requirements:
- Current RN license for the state in which the nursepractices in
- One (1) year nursing experience in a long-term acute carefacility preferred
- Experience in acute rehabilitation for vents, wound care,cardiac, and stable post trauma patients
- Complies with all relevant professional standards ofpractice
- Participation and completion of Amergis’ Competency programwhen applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable tojob site
- Understand patient confidentiality and HIPAArequirements
- Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
- Computer proficiency required
