The Long Term Care LPN assumes responsibility andaccountability for the delivery of care for patients in a Long TermCare/Assisted Living Facility. The nurse demonstrates ability to make clinicaljudgments in an effective and efficient manner with supervision, as well ascritical thinking and performance ability in the coordination of patient care.Additionally, this individual performs according to nursing standards and isaccountable in managing patient care and assisting others in the management ofpatient care.
Essential Duties and Responsibilities:
- Utilizes the nursing process to assess, plan, implement andevaluate patient care in a Long Term Care/Assisted Living Facility
- Performs focused interview to identify specific needs
- Assess signs and symptoms indicating physiologic andpsychological changes in the patient’s condition
- Collects, analyzes, and interprets data and information fromhealth care members and documents actual and/or potential nursing diagnoses
- Documents the patient’s plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventions
- Identifies and prioritizes actions based upon patient carerequirements
- Monitors consistency between patient plan of care andactions of medical staff
- Performs interventions according to identified priorities,plan of care, and the facility policies and patient outcome standard
- Provides a safe, comfortable and therapeutic patientenvironment
- Initiates and evaluates patient and/or family education
- Revises the plan of care according to evaluation, changes inmedical plan of care, and effective/ineffective nursing interventions
- Document’s patient’s response to nursing and otherinterventions
- Uses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing priorities
- Monitors safety and effectiveness of the environment andequipment
- Participates in activities to facilitate good interpersonalcommunications with the chosen facility
- Supports professional practice by observing the following:Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2)Standards of Nursing Practice; 3) Standards of Patient Care; 4) JointCommission, state and local laws; 5) Patient’s Bill of Rights; and 6) AmergisHealthcare, nursing and client hospital policies
- Performs other duties as assigned/necessary
Minimum Requirements:
- Current Licensed Practical Nurse License for the state inwhich the nurse practices
- Ability to effectively elicit/provide information to andfrom appropriate individuals via strong spoken, written, listening, andnonverbal communication skills
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray
- One year prior professional nursing experience preferred
- Current BLS card
- Successful completion of screening tests for the clinicalspecialty area(s) in which the LPN practices with a score of 80% or greater
