We are seeking a Clinician for a non-profit geriatric center in San Francisco, Ca!  

The facility is a community-focused organization in the San Francisco Bay Area dedicated to helping geriatric patients maintain their health, activity levels, and social connections. The Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention.  The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span. 

Essential Duties and Responsibilities:

  • Crisis Intervention and Management: Provide crisis intervention and management to prevent hospitalization.
  • Participant Assessments: Conduct intakes and assess participants with psychiatric, psychological, and/or behavioral management needs.
  • Therapeutic Services: Offer individual and/or family therapy for participants, their families, or caregivers.
  • Case Management: Deliver case management services as needed, including intensive case management, evaluations, and service plan development.
  • Team Coordination: Collaborate with members of the Interdisciplinary Team.
  • Group Therapy: Lead therapy groups for participants who can benefit from group therapy.
  • Team Contributions: Participate in team meetings and case conferences by presenting cases, discussing clinical issues, and exchanging information and ideas.
  • Community Resources: Provide information about community resources to the general public as needed.
  • Supervision: Provide direct supervision to LCSW or MFT license-eligible staff.
  • Program Development: Assist psychologists in developing and implementing programs (e.g., behavior management programs) and policies to support the Mental and Behavioral Health Team.
  • Quality Control: Help psychologists supervise mental health team staff and ensure quality control for mental health services.
  • Staff Training: Assist psychologists in assessing staff training needs and creating appropriate training programs and materials in mental health.
  • Training Administration: Help plan, develop, and administer staff training seminars and materials.
  • Additional Duties: Perform all other reasonably related duties as assigned.

Minimum Requirements:

  • Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education required
  • Active license as an LCSW in state of practice required
  • Minimum of one year of social work experience preferred
  • Complies with all relevant professional standards of practice 
  • Participation and completion of Amergis’ Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Successful completion of new hire training as applicable to job site 
  • Understand patient confidentiality and HIPAA requirements  
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required  
  • Computer proficiency required 
  • Must be at least 18 years of age
