HOSPICE RN 

Location: Worcester

Start Date: ASAP

Duration: 13 weeks 

Shift: 40 hours/week(8a-4:30p) or 36 hours/week (7p-7a)

Requirements: 

  • 2+ years of Hospice RN Experience 
  • OASIS Experience 
  • Homecare/Homebase strongly preferred 

PAY:

  • Local Rate: $68/hour
  • OPEN TO TRAVEL

The Registered Nurse – Hospice is responsible for providing care, comfort and support to terminally ill patients.  The Registered Nurse – Hospice assumes the responsibility and accountability for the delivery of hospice patient care in various settings.  The Registered Nurse demonstrates the ability to make clinical judgments in an effective and efficient manner with supervision.  The Registered Nurse will also demonstrate critical thinking and performance ability in the coordination of patient care.   Consistently performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.

Essential Duties and Responsibilities:

  • Utilizes the nursing process to assess, plan, implement and evaluate patient care
  • Performs focused interview to identify specific patient needs
  • Assesses signs and symptoms indicating physiologic and psychosocial changes in patient’s condition
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses
  • Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
  • Identifies and prioritizes actions based upon patient care requirements
  • Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard
  • Provides a safe, comfortable and therapeutic patient environment
  • Initiates and evaluates patient and/or family education and provides guidance and support
  • Revises plan of care according to evaluation, changes in medical plan of care, effective/ineffective nursing interventions
  • Documents the patient’s response to nursing and other interventions
  • Uses clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing priorities
  • Monitors safety and effectiveness of the environment and equipment
  • Advises grieving family members on coping techniques and community resources
  • Performs other duties as assigned/necessary

Minimum Requirements:

  • Current Registered Nurse license for the state in which the nurse practices.
  • Complies with all relevant professional standards of practice 
  • Participation and completion of Amergis’ Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Successful completion of new hire training as applicable to job site 
  • Understand patient confidentiality and HIPAA requirements  
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required  
  • Computer proficiency required 
  • Must be at least 18 years of age
