Community Resource Coordinator.

Amergis Healthcare Staffing is searching for an enhanced case management role for the newly proposed Lifespan waiver program. We are seeking Community Resource Coordinators (CRC) who have a passion for supporting individuals with intellectual and developmental disabilities and autism to engage in a meaningful Person-Centered Planning Process. 

13 paid holidays per year

Up to 10 days of accrued PTO, based on assignment duration

Please note that this pay range represents a good faith estimate of the compensation that will be offered for this position. The actual pay offered to a successful candidate will take into account a wide range of factors, including but not limited to location, qualifications, and additional preferred experience.

Knowledge, Skills and Certifications:

Minimum Requirements for Community Resource Coordinator:

  • Bachelor’s Degree from an accredited educational institution in Social Work, Psychology, Education, Human Development, Nursing, Rehabilitation, or directly related human services field and a minimum of two (2) years’ experience in providing support services (paid or unpaid) to clients/families with disabilities or providing support services in a directly related human service field.
  • Ability to drive, access to vehicle for travel, and ability to work from home and maintain HIPAA-compliant home office is also required
    • Preference will be given to applicants who meet the minimum requirements and also have: lived experience as an individual with a disability or an immediate family member of an individual with a disability; experience providing support (paid or unpaid) to individuals who have intellectual or developmental disabilities or autism; extensive knowledge of, and connections to, local community organizations, people and resources, including employers; linguistic and cultural competencies, including applicants who are Deaf or hard of hearing; a Master’s Degree in the above listed degrees; or licensure as a Licensed Social Worker (LSW).

Job Description:

Responsibilities of the Community Resource Coordinator include:

  • Conduct face-to-face intakes with Individuals who have been determined eligible for the Office of Aging and Disability Services (OADs) Developmental Services program.  CRCs may also complete assessments to assist with eligibility for Waiver programs in sections 21, 29, and 20.   
  • Develop a strength-based, person-centered, and outcome-based Person Centered Plan (PCP) with the individual and their chosen team, that includes important details and actions to prevent, reduce or resolve barriers for implementation and follow-up with the individual and their team.
  • Identify and outline interventions to facilitate the individual’s desired outcomes and clinical and functional goals that will support outcomes.
  • The Community Resource Coordinators monitor the effectiveness of the psycho-social related interventions through a dynamic and fluid process that requires modification of the PCP as needed, based on individual needs.
  • The Community Resource Coordinators and their Manager will serve as part of a Transition Team and will meet quarterly to review caseload trends. These outcomes will be reported to leadership and help inform the development of the Lifespan Program, ensuring adequate resources are available to those we serve.

Schedule: Routine Business Hours M-F, 40 hours a week, Telework eligible position

Additional Information: A Coordinator is needed for each Public Health District in the state of Maine. The Districts Needing support are York County and Waldo County, Maine
