The Long-Term Acute Care LPN provides the delivery of directand indirect patient care to medically complex patients under the clinicalsupervision of the Registered Nurse in facilities or specialty care hospitals.
Essential Duties and Responsibilities:
- Provides delivery of direct and indirect patient care
- Responsible for providing care to medically complex patients
- Monitors and documents patient status and condition whichmay include vital signs, drains, dressings, and lab values
- Informs appropriate Registered Nurse/Physician of activitiesperformed, observations of patients and their responses, and changes in patientstatus
- Administers and documents administration of medications perfacility policy
- Documents all activities related to the patient’s careaccording to the patient’s plan of care
- Maintains confidentiality at all times with patients,medical staff and co-workers
- Provides a safe, comfortable and therapeutic patientenvironment
- Experience in acute rehabilitation for vents, wound care,cardiac, stable post trauma patients
- Adheres to and supports thepolicies/procedures/goals/objectives of the hospital in an attempt to providequality patient care
- Performs interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standard
- Identifies and prioritizes actions based upon patient carerequirements
- Performs other duties as assigned/necessary
Minimum Requirements:
- Current nursing license for the state in which the nursepractices in
- One (1) year nursing experience in a long term acute carefacility preferred
- Complies with all relevant professional standards ofpractice
- Participation and completion of Amergis’ Competency programwhen applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable tojob site
- Understand patient confidentiality and HIPAArequirements
- Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
- Computer proficiency required
