Job Title: Physical Therapist (PT) 
Location: Kokomo, IN
Schedule: Monday–Friday, 8:00 AM – 4:30 PM (No weekends or holidays)
Hours: 40 hours/week
Pay: Competitive hourly rate based on experience- $60/hour

Overview:
We are seeking a compassionate and skilled Physical Therapist (PT) to join our team in Kokomo, Indiana. This full-time role offers a consistent weekday schedule with no weekend or holiday shifts, making it ideal for professionals seeking work-life balance in a rewarding clinical setting.

Responsibilities:

• Perform patient evaluations and develop individualized treatment plans
• Provide and document therapy services in accordance with physician orders and care plans
• Collaborate with interdisciplinary teams to support patient outcomes

Requirements:

• Current Indiana PT license
• Graduate of an accredited Physical Therapy program
• Minimum 1 year of experience working with the geriatric population
• Current CPR certification (if applicable)
• TB Questionnaire, PPD or chest x-ray (if applicable)
• Current health certificate (per contract or state regulation)
• Must meet all federal, state, and local requirements
• Must be at least 18 years of age
