Clinical Case Manager – Temporary Assignment (ASAP through January 12)
?Location: Onsite in Escondido, CA
Compensation: $35–$40 per hour
Schedule: Monday–Friday, 8:00 AM–5:00 PM

Position Overview This temporary role supports at-risk Veterans recently discharged from hospitals through the Recuperative Care Program. The Clinical Case Manager will provide trauma-informed case management to help clients regain independence. Responsibilities include coordinating care plans, connecting clients to community resources, supporting housing and recovery efforts, and providing crisis intervention. The role also involves collaboration with VA staff, maintaining accurate documentation, and occasionally transporting clients to appointments.

Qualifications

  • Master’s degree in Social Work (MSW) or Marriage & Family Therapy (MFT)

  • Registered Associate Social Worker (ASW) or Associate Marriage & Family Therapist (AMFT) in California

  • License-eligible (must obtain licensure within six months)

  • Minimum of one year of relevant experience

  • Knowledge of substance use disorders preferred
