Licensed Practical Nurse (LPN)– Pediatric Long-Term Acute Care
Location: Centralia, WA 98531
Assignment Duration: 13–26 weeks (with extensions available)
Schedule: 3x12s, NOC shift, 6:00 PM – 6:30 AM OT available
Compensation
Local Pay Package:
- Hourly Rate: $41/hour | OT rate: $61.50/hour
- Weekly Total (36 hours): $1,476 (taxable)
Travel Pay Package:
- Taxable Base Pay: $21/hour | OT rate $63/hour
- Weekly Taxable Total (36 hours): $756
- Non-Taxable Stipends:
- Meals: $476/week
- Housing: $770/week
- Total Weekly Gross: $2,002
Job Summary
This position provides direct andindirect nursing care to medically complex pediatric and young adult residentsin a long-term acute care setting. The LPN works under the supervision of an RNand collaborates with an interdisciplinary team to assess, plan, and implementindividualized care plans. Responsibilities include medication administration,health monitoring, family support, and CNA supervision.
Requirements
Licensure:
- Active LPN license (WA or Compact)
Experience:
- Minimum 1 year of pediatric experience
- Ventilator and tracheostomy care experience (required)
Key Responsibilities
- Conduct systematic assessments and develop individualized care plans
- Administer medications and treatments per care plan
- Monitor and document changes in resident condition
- Accompany residents to medical appointments and coordinate care
- Maintain accurate health records and ensure HIPAA compliance
- Provide oversight and guidance to CNAs
- Participate in care planning and interdisciplinary collaboration

- Proficiency in vent/trach care, pediatric, and DDA experience
- Strong communication and emotional resilience

- Physical stamina (lift up to 40 lbs, long periods of standing/walking)
