- Amergis Healthcare is seeking an experienced Clinical Assessment Registered Nurse (RN) to support behavioral health clients through in-person assessments across the Boston–Cambridge area. This role is part of the Assessment Unit, which is responsible for conducting initial and ongoing face-to-face assessments for CCA enrollees, ensuring accuracy, quality, and timely submission to MassHealth.
- You will work collaboratively with care teams to identify member needs and support interventions that improve health outcomes. This position reports to the Regional RN Supervisor.
- Conduct in-person assessments and clinical exams, including medication reconciliation, vital signs, and chronic disease education.
- Administer behavioral health screeners (e.g., PHQ-9, EASI, AUDIT, DAST).
- Escalate urgent member needs to supervisors as appropriate.
- Document assessments using electronic health records and care management platforms.
- Identify unmet needs and collaborate with care teams to develop individualized care plans.
- Provide telehealth assessments when clinically appropriate.
- Ensure timely referrals to behavioral health and long-term support services.
- Maintain compliance with organizational policies and confidentiality standards.
- Participate in quality improvement initiatives and ongoing clinical education.
- Travel required within the Boston–Cambridge area (occasional travel near the NH border may be requested but is highly unlikely).
- Assessments conducted in residential and community settings.
- Must have a valid driver’s license and reliable transportation.
- Associate Degree in Nursing (ADN)
- Active RN license in Massachusetts, in good standing
- 1+ year of clinical experience (including clinical rotations)
- Experience with MDS (Minimum Data Set) assessments
- Intermediate to advanced Microsoft Office skills
- Proficiency with electronic health records
- Strong clinical judgment and assessment skills
- English fluency
- Bachelor’s Degree in Nursing (BSN)
- Experience in care management
- Bilingual in English and a second language
