Job Title: Physical Therapist (PT) –Rehabilitation Center

Location: Raleigh, NC

Job Type: Full-Time

Schedule: 40 Hours/Week – OccasionalWeekends

Contract Length: 13 Weeks

Job Description:

We are seeking a compassionate and skilledPhysical Therapist to join our team in a long term care/rehabilitation centersetting. This role is ideal for a dedicated professional who is passionateabout improving the mobility, independence, and quality of life for elderly andrehabilitative patients.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Deliver therapeutic interventions including exercises, manual therapy, and mobility training.
• Collaborate with an interdisciplinary team including physicians, nurses, and other therapists.
• Monitor and document patient progress, adjusting care plans as needed.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Maintain compliance with all regulatory and documentation standards.

Qualifications:

• Active Physical Therapist license in North Carolina
• Minimum 1 years of PT experience
• Degree from an accredited Physical Therapy program
• Current CPR certification
